Weekends & bank holidays
An Updated CV: Full employment history, and education. Please justify any gap more than 6 months.
References: Please provide 2 references from senior Band of current/past employment.
Passport: Full and colour copy of your current valid passport along with front cover
Right To Work: Full and colour copy of your Resident Permit, that shows your right to work, if applicable.
Proof of Address: i.e. Driving License (front and back of card), Bank Statement, Utility Bill, Council Tax Bill – dated within last 6 months. At least two of any of these.
Enhanced DBS: Please send us the copy of your enhanced DBS certificate, which is registered with update service, along with your permission to check it online
Practical Training: Please provide basic Life Support, Manual Handling & PMVA, MAPA(For mental health work) practical courses certificates( if Possible)
Mandatory Training: Please send us the copy of the certificate. If you do not have this, please let me know so that I can setup online trainings to you.
Indemnity Insurance and Union: Please send us copy of card or any letter issued by them. if you do not have this, please declare
Company Certificate: copy of company certificate for ltd companies’ candidate
Photo: Passport size photograph for your ID Badge
Occupational health Questionnaire: Please download this attachment, complete and (*Please note – Hand signature is must)
Serology / Immunity: Measles, Rubella, Varicella (this can be self-declared if you have had chickenpox in the past), TB (BCG Scar) and a Hep B Titre Reading. EPP results for Hep B Ant, Hep C, HIV
